After you Print out the Agreement for your records and have Read the full Independent Contractor Agreement please electronically sign by filling out the below form.

• Fill out your complete Owner's First and Last Name along with your current Email Address.
   
• Provide any Additional Email Addresses that you have so we can contact you at.
   
• Provide your 7 digit Internal Agent ID # ~ ex. 90xxxxx.  This number can be found when you sign into the site.  Do not enter your IATA number (if you have one). 
  
  If you have recently signed up, we will need to first activate your account in order for you to receive your Agent ID # and therefore you will have to WAIT before filling out the agreement.
   
• Provide your Agency Name and full Agency Business Address.
   
• The reason we ask for your cell number is so we can get in touch with you if there is an emergency with a vendor.  We will not give out this number as it is only used for internal use.
   
• Please list the Full Name(s) of any other agents that you have working for your agency.  Make sure that they are also listed in your profile in the agent site and this can be done by clicking on Edit Your Profile once you are log into your account.  Sometimes a vendor will contact us and will be looking for that agent.
   
• Indicate that you Read Each Page of the agreement by clicking on the drop down menu clicking on the button labeled Yes.
   
• Finally enter Today's Date & Your Initials to sign off & execute the agreement.

This should be filled out by the Owner of the Agency.  The form below will not retain your information once you click on the Submit Form.

Once you click on the Submit Form button above you will be taken back to this form.

If you do not have an Agent ID # provided by us, then please do not fill out the form as we must activate your account.

Please DO NOT fill out this form again as you should receive an email confirmation that we have received your electronic signature.
